Job Description
MD24 House Call is currently seeking a compassionate, patient focused Physician to provide initial (MDQA) and urgent visits to patients in private homes, group homes, independent, assisted and skilled living facilities. They may also see patients in our Chronic Care Management Centers in person or via Telemedicine visits. The MD/DO will be responsible for conducting in-home visits in an assigned geographic area of responsibility and supervise up to four advanced practice providers.
